After repetitive requests for the LyX file (sent individually via E-mail), I decided that it was rather time consuming. So with hesitance, I am more than willing to take my chances and make the file public. The file comes with content which serves merely as a placeholder (it's an old, 'quick-and-dirty' continuation report), but some of it is work that I intend to publish.

You can download the file (in some Web browser you will need to "Save page"), open it, and then use it as an example or a starting point (deplete-and-replace). I beg you to treat the file with respect. It contains plenty of personal comments that I did not bother to remove. If you have any problems with the file, please do let me know.
