No I don't think it is those two. If you look at the error it is an 'uncorrectable error' - bad cables normally give either CRC errors (especially on UDMA) or timeouts. Kernel screw ups are normally 'Busy' or timeouts.

third , the disk is dying

Coupled with the SMART stuff talking about sectors pending for remapping it is bad news.
